The Initial Visit
Our compassionate pediatric providers do whatever it takes to make every visit to our office a pleasant, relaxed experience. With all of the important information about our practice available on our website, you can feel confident that you and your child are well-prepared for your first appointment. We also invite you to review our about us page in order to get to know our clinicians and staff before you come in. We look forward to meeting you!
Prior to your first appointment please send or drop off:
- Complete immunization records
- Medical records from your previous pediatrician
For your first appointment, please print and fill out the New Patient Forms , and bring them with you, along with your insurance card and any questions or concerns for your provider.
Please plan on arriving 15 minutes early so your child can play in our playroom while we assure your paperwork is in order. These steps allow you to have the full time with your provider, and your child to get comfortable with our office prior to the appointment.
What to bring to your appointment:

- Patient Forms
- List of any questions and concerns for provider
Please be ready to tell provider:
- Child's medical history, medications, over the counter medications and supplements and any allergies
- Details about your family medical history
- Recent care obtained outside our practice
- Info on how your child is doing at home, school, etc
